2012 CCHD LOCAL GRANTS

Thank you for your interest in the CCHD LOCAL Community or Parish Grant Program.

In order to obtain an application for a CCHD Local Community or Parish Grant, you must review an informational program and then complete and return the CCHD Local Grant Application Survey. (This process takes approximately 30 minutes.)  Once you complete and return the survey, the CCHD Local Community or Parish Grant Application will be made available to you.

The purpose of this informational program is to present the specific objectives and purpose of the CCHD Local Grants and define the criteria necessary to qualify for a CCHD Local Grant.

We have found that applicants who understand the specific objectives, purpose and criteria of the CCHD Local Grant Program, are able to complete an application that is much more likely to qualify for one of the CCHD Local Grants.
