I. CONSENTIMIENTO DE PARTICIPACIÓN Entiendo y certifico que mi participación, o la de mi hijo, en las actividades en Camp Twin Lakes es completamente voluntaria. Me he familiarizado con el programa y las actividades de RCAA Administration Services, Inc., en Camp Twin Lakes, en las que mi hijo/yo participaremos. Reconozco que ciertos riesgos y peligros son inherentes a estas actividades, que pueden incluir, entre otras, las actividades de equitación, cursos de cuerdas desde lugares altos y bajos, natación, tiro con arco, jardinería, cocina, ciclismo, deportes, actividades en el lago y canotaje. Reconozco que aunque RCAA Administration Services, Inc., y Camp Twin Lakes han tomado medidas de seguridad para minimizar el riesgo de lesiones a los participantes del campamento, RCAA Administration Services, Inc., y Camp Twin Lakes no pueden asegurar ni garantizar que los participantes, el equipo, las instalaciones o actividades estarán libres de peligros, accidentes o lesiones. Reconozco y he instruido a mi hijo sobre la importancia de conocer y cumplir con las reglas, regulaciones y procedimientos del programa de RCAA Administration Services, Inc., en Camp Twin Lakes. Además, doy fe de que mi seguro de salud cubrirá todos los gastos médicos y de hospital en los que yo o mi hijo incurramos y que he recibido la aprobación de un médico autorizándo a mí o a mi hijo a participar en las actividades de RCAA Administration Services, Inc., en Camp Twin Lakes. También acepto informar a RCAA Administration Services, Inc., de cualquier actividad en la que yo o mi hijo no podamos participar. Entiendo y acepto que mi hijo estará en un ambiente que involucra elementos relacionados con la naturaleza, acampar o vivir en comunidad, como insectos y picaduras de insectos, exposición al sol o enfermeda-des contagiosas.
II. RENUNCIA DE RESPONSABILIDAD Yo, el abajo firmante, entiendo que ocasionalmente ocurren accidentes durante las actividades del campamento y que los participantes pueden sufrir lesiones personales graves y daños a la propiedad como consecuencia de ello. No obstante, sabiendo los riesgos de las actividades del campamento, acepto asumir esos riesgos y, al firmarlo, libero de toda responsabilidad a RCAA Administration Services, Inc., y Camp Twin Lakes, y asumo la intención de responder legalmente, y la de mis hijos menores, mis herederos, albaceas y administradores. Por medio de la presente libero y absuelvo para siempre a RCAA Administration Services, Inc., Camp Twin Lakes y cualquiera de sus clérigos, pastores, voluntarios, funcionarios, directores, empleados, socios, accionistas, miembros de la junta, servidores, agentes y cesionarios de y contra todos los reclamos, causas de acción, daños, pérdidas y/o gastos que surjan o se relacionen con cualquier lesión, enfermedad o pérdida de cualquier tipo, conocida o desconocida, incluidas, entre otras, lesiones a la propiedad, a la persona, a mí o a mi hijo/pupilo, durante mi asistencia o la de mi hijo/pupilo al programa de Servicios Administrativos de RCAA, Inc., en Camp Twin Lakes, o en relación con la misma.
III. AUTORIZACIÓN PARA MEDIOSYo, autorización a RCAA Administration Services, Inc., y Camp Twin Lakes para entrevistar y/o tomar fotografías, grabaciones de audio o audiovisuales míos o de mi hijo para usarlas en materiales promocionales, educativos o de recaudación de fondos, incluidas, entre otras, cintas de video, folletos y panfletos. Entiendo que mi nombre o el de mi hijo puede usarse en relación con estos materiales. Al firmar este documento, asumo la intención de responder legalmente, y la de mis hijos menores, mis herederos, albaceas y administradores. RCAA Administration Services, Inc., y Camp Twin Lakes tendrán derecho a utilizar fotografías u otras imágenes mías o de mi hijo en materiales de promoción, educativos o de recaudación de fondos. Reconozco que RCAA Administration Services, Inc., o Camp Twin Lakes tendrán todos los derechos de autor sobre dichas fotografías y cintas de video y pueden utilizar dichos derechos de autor en su totalidad. También, por medio de la presente, libero a RCAA Administration Services, Inc., y Camp Twin Lakes y sus funcionarios, agentes y empleados de toda
responsabilidad relacionada con la toma y el uso de estos materiales según lo autorizado por RCAA Administration Services, Inc., y Camp Twin Lakes. Además, renuncio a todos los derechos, intereses o reclamos de pago en relación con cualquier exhibición o publicación de estos materiales. I. PARTICIPATION CONSENT I understand and certify that my participation in the and its activities at Camp Twin Lakes is completely voluntary. I have familiarized myself with the RCAA Administrative Services, Inc. program and activities at Camp Twin Lakes in which I will be participating. I recognize that certain hazards and dangers are inherent in these activities, which may include, but not limited to, the activities of horseback riding, high and low elements ropes course, swimming, archery, gardening, cooking, biking, sports, lake swimming, and boating. I acknowledge that although RCAA Administrative Services, Inc. and Camp Twin Lakes have taken safety measures to minimize the risk of injury to camp participants, RCAA Administrative Services, Inc. and Camp Twin Lakes cannot insure or guarantee that the participants, equipment, premises, or activities will be free of hazards, accidents, or injuries. I recognize and have instructed myself in the importance of knowing and abiding by the rules, regulations, and procedures for the RCAA Administrative Services, Inc. program at Camp Twin Lakes. Further, I attest that my health insurance will cover any medical and hospital expenses that I incur, and I have received approval from a doctor authorizing me/my child to participate in RCAA Administrative Services, Inc. activities at Camp Twin Lakes. I also agree to inform RCAA Administrative Services, Inc. of any activities in which I/my child may not participate. I understand and agree that I will be in an environment that involves elements related to nature, camping or community living, such as insects and insect bites, sun exposure, or communicable illnesses.
II. LIABILITY RELEASE I, the undersigned, understand that occasionally accidents occur during camp activities and that participants may sustain serious personal injury and property damages as a consequence thereof. Knowing the risks of camp activities, nevertheless, I agree to assume those risks and by signing this liability release, I intend to legally bind myself, my minor children, my heirs, executors, and administrators. I hereby release and forever discharge RCAA Administrative Services, Inc. and Camp Twin Lakes, and any of their clergy, pastors, volunteers, officers, directors, employees, partners, shareholders, board members, servants, agents, and assigns from and against all claims, causes of action, damages, losses, and/or expenses arising out of or relating to any injury, illness, or loss of any kind, known or unknown, including but not limited to injuries to property or person, to me during or related to my attendance in the RCAA Administrative Services, Inc. program at Camp Twin Lakes.
III. MEDIA RELEASE I do give RCAA Administrative Services, Inc., Camp Twin Lakes, and all authorized representatives of Camp Twin Lakes the right to interview and/or to take photographs, audio or audio-visual recordings of me to be used in promotional, educational or fundraising materials including, but not limited to videotapes, pamphlets and brochures. I understand my name may be used in connection with these materials. By signing this media release, I intend to legally bind myself, my minor children, my heirs, executors, and administrators. RCAA Administrative Services, Inc., Camp Twin Lakes, and all authorized representatives of Camp Twin Lakes shall have the right to use photographs or other images of me in promotion, educational or fund-raising materials. I acknowledge that RCAA Administrative Services, Inc., Camp Twin Lakes, and all authorized representatives of Camp Twin Lakes shall have all rights of copyright in and to such photographs and videotapes and may use such copyright fully. I also hereby release RCAA Administrative Services, Inc., Camp Twin Lakes, and all authorized representatives of Camp Twin Lakes including its officers, agents and employees from all liability connected with the taking and use of these materials as is authorized by RCAA Administrative Services, Inc., Camp Twin Lakes, and all authorized representatives of Camp Twin Lakes. In addition, I waive all rights, interest or claims for payment in connection with any exhibition or release of these materials. This consent is voluntary, and I give it in the interest of public information, education, the furtherance of the goals of these institutions, or other lawful purposes. I acknowledge that I have legal authority to sign this form on behalf of the minor whose name is mentioned above.
