All priests are required to request a letter of suitability from the Office of Priest Personnel for any ministry that is done outside the Archdiocese of Atlanta. Please fill out the form below. Please note you must complete ALL fields.  Any forms that do not have the completed information will be returned to you.

When requesting a letter of suitability, it is important to do so in a timely manner. Please do so at least one month in advance. The only requests considered emergencies are funerals. In which case, please contact Yolanda Rivera at 404-920-7329 or by email.
