The primary use is to provide current copies of forms that are used in marriage preparation.

Each complete marriage file should have completed Bride/Groom/Registration forms and appropriate Affidavits. You are asked to avoid making copies of these forms for future use as they will be changed on occasion and by downloading the forms below each time one is required, you will be assured of having the “current” version.

Verification of Marriage Preparation Hours For The State of Georgia

This form may be helpful to you in documenting your work with engaged couples for the State of Georgia. Georgia law provides a discount on the marriage licenses of those couples who can provide proof that they have received a minimum of six (6) hours of marriage preparation. Some counties require that their own form be used. Many counties accept a note on parish letterhead.
